Here is a screen shot of the Age-Friendly 4Ms Health Factor template. This is just the top portion of the template but the part that includes the ‘what matters’ health factor as well as optional patient priorities care health factor.
This example is showing if / when you should not only select ‘yes’ for the ‘what matters’ box but when you should consider selecting the first ‘Patient Priorities Care’ health factor or ‘identify’ health factor. This is appropriate if you are able to clarify and document a specific and realistic health outcome goal that is connected to what matters.
The second Patient Priorities Care health factor is appropriate to select if you are taking steps to align care based on that person-centered goal.
