The Patient Priorities Care Mentored Partner program has been supported by the Office of Geriatrics and Extended Care since FY2020 as a way to enhance the quality of ‘what matters’ conversations for older adults across VHA. It provides operational funding to clinical teams throughout VHA to implement PPC as part of a community of practice.

• 2-year funded opportunity for PPC implementation at clinical sites throughout VHA
• Funding provided by the Office of Geriatrics and Extended Care
• Monthly meetings supporting PPC training and implementation
• Collaboration with other VA sites

Applications are typically available late spring/early summer.

Ways to prepare for becoming a Mentored Partner:

• You will be added to our private Teams channel that contains many of our resources and a direct line to the mentor team
• We encourage you to explore our Training options to familiarize yourself with PPC
• Share information about PPC with potential clinical settings/teams that might be interested (see our website for tips on Implementation planning and Training)
• Start to consider where and how you might integrate PPC into your clinical setting, the support you might need, and how you might use funding to support clinician time or supplies (see our implementation partner matrix for more information)
