Veteran Health Priorities Self-Assessment

Patient Priorities Care-informed clinical visits that focus on what matters vs. “what’s the matter with me” can be a new experience for Veterans and their care partners. Therefore, experienced members of the PPC community have developed tools to help prime conversations about what matters that are rooted in PPC core principles.

  • Veteran Priority Identification Self Assessment Worksheet: This tool can be provided to Veterans or their care partners prior to a visit where PPC is practiced. It can also help explore values, preferences, and priorities and give a clinician extra insight before the health care encounter.

The Veteran Health Priorities Self-Assessment is a collaborative project involving health professionals and Veteran Engagement Groups from Puget Sound Geriatric Research Education and Clinical Center (GRECC), James J. Peters GRECC, Philadelphia and Michael DeBakey VA Healthcare Systems. Participants of the Office of Geriatrics and Extended Care PPC Mentored Partnership Program provided additional feedback.
